| The US Business Do Not Call Registry, operated by National Business Registry, LLC. (“NBR”) 16805 US Highway 19 North, Clearwater, Florida, 33764 (“service providers address”) will provide you with the opportunity to limit the number of unwanted telemarketing calls your business receives in accordance with the terms and conditions of the U.S. Do Not Call Registry as set forth below. |

| The words “you and “your” means the business who has agreed to accept the offer to limit the number of unwanted telemarketing calls and who accepts the terms and conditions at the time of the sale or any time thereafter. By accepting the terms and conditions you agree to all of the provisions of the terms and conditions. The words “we”, “us” and “our” mean the U.S Business Do Not Call Registry. |

| I. The Offer |
| The U. S. Business Do Not Call Registry was designed to provide you with the opportunity to limit the number of unwanted telemarketing calls your business receives. By registering your business telephone number(s) we will make outbound notification(s) to telemarketing companies to delist your number. Your Registration covers up to five of your current and active businesses telephone lines. If your registered telephone number changes you can notify us via our website www.USBusinessDoNotCallRegistry.com or by calling Customer Service 877-9-DoNotCall (877-936-6682) |

| II. How the Service Works |
| Telemarketing companies are contacted by the U.S. Business Do Not Call Registry through direct mail, telephone, internet and trade show presentations. These companies are given access to the database for the sole purpose of removing business telephone numbers from their calling lists. Telemarketing companies may use the U.S. Business Do Not Call Registry to prescreen their leads and remove the phone numbers from their calling lists. |

| III. Rates and Billing Terms |
| The U.S. Business Do Not Call Registry is billed to your local telephone bill on the CIC page as being billed on behalf of National Business Registry or Personal Voice, Inc depending on your coverage area. All customers must be at least 18 Years of age and authorized to make changes to and incur charges on the primary telephone account provided. |

| IV. Service Termination / Suspension |
| To cancel services contact the U.S. Business Do Not Call Registry Customer Service department between the hours of 8 am and 8 pm EST at 877-9-DoNotCall (877-936-6682) All Customers will be given ninety days of free service to continue measuring the benefits of your membership before the service is effectively cancelled. If you do not call within ninety days to cancel the service we will resume billing for the service after that time.
Service will be terminated immediately if we are unable collect payment due to LEC (Phone Company) restrictions and / or any other reasons. U.S. Business Do Not Call Registry reserves the right to continue service and mail a bill if we are unable to collect via LEC. If payment is not received within the time frame stated on the bill the service may be permanently cancelled. |

| V. Changes to Service |
| The U.S. Business Do Not Call Registry reserves the right to make changes to fees, terms & conditions and general policy and procedures without prior notice to the Customers. |

| VI. Reporting Telemarketing Calls |
| As a Registry Member you are agreeing to contact us and report any unwanted telemarketing calls to your business. U.S. Business Do Not Call Registry representatives may contact the telemarketing companies you report and give them an opportunity to sign up for access to the Registry. If the telemarketing company has already received access to the Registry they may be contacted for violating the terms of use agreement and enforcement actions may be taken. We reserve the right to keep an internal log of the complaints made for each telemarketing company and may forward them to the FCC on your behalf when necessary. When reporting unwanted telemarketing calls, Members must provide the call date and time, name and telephone number of the marketing agency and any other information that is requested by our representatives or website. Failure to provide the proper information may delay the processing of your complaint or prevent it from being properly forwarded. |

| VII. Limitation of Liability |
| The U.S. Business Do Not Call Registry will make every attempt to limit the number of telemarketing calls your business receives. This may take up to 45 to 60 days. If you have previously given permission to telemarketers and / or have an established business relationship with them we can not guarantee that all calls to your business will be stopped. Members of the U.S. Business Do Not Call Registry agree not to accept offers made by telemarketing companies or sent through junk faxes to your business. If you have an existing business relationship with a telemarketing company National Business Registry, LLC is not responsible for contacting them if you continue to their accept offers as a Member of the Registry. |

| VIII. How your Business Information is used |
| The information you provide us is used to fulfill your request for service and notify you of any changes in the status of your account. We also use it to inform third party businesses and direct marketers to immediately remove your business phone and fax numbers from their calling list. In order to honor your request not to be called, telemarketing companies will be given permission to use the numbers in our data base in their calling list validation processes for the purpose of removing it from their calling list. Any other use is a violation of the terms of use agreement and is subject to compliance and / or enforcement actions. |
